Know where each fact came from.

ForMyMedical should distinguish patient-entered notes, clinician-confirmed information, imported records and unverified data. A convenient browser screen must never silently convert a note into a medical fact, consent, diagnosis or professional instruction.

Safety boundary

DEV prototype only. It does not diagnose, prescribe, replace emergency services, create clinician authorization, or prove consent. Production use with real health data requires validated privacy, security, retention, audit, consent and regulatory controls.

4 • SOURCE CONFIDENCE

Every important item carries its trust label.

Clinician-confirmed

Show who confirmed it and the confirmation date. A later patient edit becomes a separate note until reconfirmed.

Imported record

Keep source organization, document date and import date visible so provenance is not lost.

Patient-entered

Useful personal context stays clearly labeled as self-entered rather than silently becoming a verified medical fact.

Needs review

Conflicting, stale or source-missing information is surfaced for review instead of being guessed or automatically merged.

Trust rule: newer does not automatically mean more authoritative. Source, date and verification state travel together, and uncertainty remains visible.
